I found this in a stack of Meteorite magazines that I purchased. Apparently, it was a premium given to new subscribers by the publisher. I'm not exactly sure when it was issued but I recall that I found it somewhere around the 2004 level in the stack.

I also received such a gift, but mine was a nice unclassified NWA slice. Ron Hartman's certificates and the desert find documentation/pictures he had for some of his meteorites were unique and still are.
